CASE


A 67-year-old female, farmer, developed nail-color changes with swelling and pain on the subungual area of her left forefinger (Figure 1). This patient often developed paronychia on her fingers. Our first diagnosis was progressed panaritium ungulate based on clinical findings and her occupation. Her left forefinger condition had not improved completely by using antibiotics medicines. CT images of her left forefinger, however, revealed a low-density space-occupying lesion (SOL) in the subungual space (Figures 2a, b). We determined that this could be a cyst structure, and that the high-density SOL in the periungual space (Figure 2c) could be inflammation. When we made a hole through her nail, abscess, and cyst wall, we could both observe and smell atheroma substances. This is a rare case of subungual epidermal cyst. There are a small number of reports about subungual cysts, although reports of CT revealing such cysts are rare.
